The portal to Queensdale is just northeast of Fort Salma, or NNW of the Meadows Waypoint.

The entrance to the Blood Witch's lair is from the top of the skull icon would be. Just south of Moogooloo village. Good luck! :)

There is a second portal by Sojourner's Waypoint that takes you into Queensdale as well, near the Godslost Swamp.

Is there a level requirement or a level I should consider getting to before jumping out of queensdale?

Level 15 would be appropriate. Although I'd recommend 16 just to be on the safer side.
